Acute pulmonary oedema during intravenous urography.

چکیده
Adverse reactions following intravenous contrast media administration are uncommon. In a study of 302,083 urographic examinations, Shehadi 1 established an overall incidence of adverse reactions to be at or slightly below 5%. Based on the clinical status of the patients and the need for treatment these adverse reactions are graded into minor, intermediate, severe and death. Minor reactions such as nausea and vomiting are common and they do not require any form of treatment. Intermediate reactions like bronchospasm require some form of treatment in the radiology department, whereas severe reactions under which acute pulmonary oedema is categorised require intensive treatment and hospitalisation.

Fatal Stevens-Johnson syndrome following urography with iopamidol in systemic lupus erythematosus.
A 46 year old white woman with active systemic lupus erythematosus developed a skin rash 8 hours after intravenous urography with the non-ionic contrast medium iopamidol. Severe Stevens-Johnson syndrome with erythema multiforme, intrahepatic cholestasis, pulmonary infiltrates and acute renal failure ensued, leading to her death. Although non-ionic contrast media are generally less toxic than tr...
متن کاملPulmonary oedema during treatment of acute water intoxication.
Acute water intoxication with deepening coma and uncontrolled epileptiform seizures in a 25-year-old previously fit male schizophrenic was treated with hypertonic (2 N) saline and a 20% mannitol solution. This improved his neurological state but precipitated severe pulmonary oedema. Intravenous frusemide increased his urinary output sufficiently to clear the pulmonary oedema. In acute water int...
متن کاملAsthma in pregnancy complicated by iatrogenic pulmonary oedema.
We report a unique case of near fatal acute pulmonary oedema developing with intravenous ritodrine, given in an attempt to suppress premature labour. The novel aspect of the case is that the patient had also been treated in the previous week with high dose nebulized beta-agonists for an episode of acute severe asthma, demonstrating that this idiosyncratic reaction to beta-adrenergic agents only...
متن کاملAcute pulmonary oedema: rare causes and possible mechanisms.
Acute pulmonary oedema usually has a fatal outcome. In this clinical report, we present rare cases of pulmonary oedema that were associated with Japanese B encephalitis, lymphangitis in breast carcinoma, fat embolism due to long-bone fracture, and the rupture of cerebral mycotic aneurysms. A total of 18 patients in the four disease categories were collected in two teaching hospitals in Taipei a...
متن کاملIntravenous isoprenaline in treatment of septic shock in man.
Intravenous isoprenaline was given to 10 patients in septic shock, of which occult myocardial failure was the main indication. Isoprenaline expedited recovery in cases of "benign hypotension," where kidney function paradoxically remained satisfactory at low systolic pressures, and was useful in cases of "cold hypotension" which were complicated by renal failure alone. No significant improvement...
